- June 25, 1971 - While taking off from South Lake Tahoe, CA, the pilot failed to account for the decrease in performance caused by density altitude. Climbing towards rising terrain the pilot attempted a steep turn, the plane stalled and crashed. (NTSB report OAK71AP055)
