  • July 12, 1991 - The owner of N88T had replaced its interior prior to this flight and in reattaching the fuel tank selector knobs, mislocated them. The aircraft suffered from fuel starvation immediately after takeoff. The pilot was killed. (NTSB Report CHI91FA208)
