- April 7, 1965 - While in cruise, the right hand propeller blade failed 30 inches from the hub and seperated, striking the left engine cowl and propeller. The aircraft was forced to land off of the Jacksonville, TX airport, where it collided with trees. (NTSB Report FTW66F0207)
